How to submit a claim

（includes hospitalization medical expenses, outpatient surgery/day surgery and pre-admission consultation/post hospitalization follow-up）

Claim required documents

• Original medical receipt
• Statement of account and doctor slip
• Discharge slip/sick leave certificate/medical certificate with clear exact diagnosis
• Medical records including: hospitalization summary, admission record, discharge summary, outpatient records and itemized breakdown of hospital charge (applicable to hospitalization in Mainland China)
• Diagnosis report and test report during hospitalization (such as pathological report, blood test report, PET Scan/CT Scan/MRI report, ECG report, ultrasound report and X-ray report etc.) (if necessary)
• Settlement advice from other insurers/ parties (applicable to claims paid by other insurers)

Remarks

• 1.The insured employee/ dependent must send the duly completed part I and part II of 《Group Hospitalization Claim Form (HK-CL-GCLA-03) 》 together with original receipts and relevant claim documents within 90 days from date of discharge/date of treatment to our company for processing. Late submissions will not be processed.
• 2.Insured employees can use e-claim service（https://gp.chinalife.com.hk）and upload claim form with medical receipts and relevant claim documents for application of hospitalization claim. For the amount of single medical receipt above HK$ 10,000，please mail us the original medical receipts and claim form within 2 working days for claim assessment.
• 3.Claim under the policy exclusions will not be processed
