Policyholder required to complete and submit the "Change of Policyholder/ Insured Personal Information/ Occupation/ Signature Form" and a certified true copy of the original identity document with the updated name, and a certified true copy of the original name change deed/notarization/other relevant information.

In addition, customers can also apply for Change of Name via OneService APP.

Policyholder are required to complete and submit the "Change of Policyholder/ Insured Personal Information/ Occupation/ Signature Form" and submit with a certified true copy of the original marriage certificate/divorce certificate/other relevant documents.

Policy status is not inforce, assigned policy, Bankrupt are not eligible for application through OneService APP.
